Teens can now access a new website, The Guide to Medical Independence, created by Healthcare Council and its partners. The accessible and health literate website is based on the printed version of A Teen’s Guide to Medical Transition, created to empower and assist young patients, including those with chronic conditions, to transition from pediatric healthcare to adult healthcare.
The new website was made possible through funding by the FISA Foundation, Health Care Improvement Foundation, UPMC Health Plan, and Healthcare Council. Healthcare Council partnered with UPMC Children’s Hospital of Pittsburgh, National Health Corps of Pittsburgh, and the Health Law Clinic at the University of Pittsburgh to develop the website, which was created and designed by Sparq Designs.
The printed version of the Guide was created by Healthcare Council and two students from Carnegie Mellon University.
Members can access the website to assist families, patients, and teens with the medical transition from pediatric to adult care.
